Complete terms and conditions for our patient rewards program

1. Program Overview

The Smile & Save Rewards Program ("Program") is a promotional offer operated by Dentists Ranked, LLC ("Dentists Ranked," "we," "us," or "our"). Through this Program, eligible participants ("Members") may receive a $60 Amazon Gift Card ("Reward") upon satisfying all Program requirements described in these terms.

By participating in this Program, you acknowledge that you have read, understood, and agree to be bound by these terms and conditions ("Terms"). If you do not agree to these Terms, you may not participate in the Program.

Dentists Ranked reserves the right to modify, suspend, or terminate this Program at any time without prior notice. Any changes will be effective immediately upon posting of the revised Terms on this page.

2. Qualifying Dental Services

The Smile & Save Rewards Program applies to self-pay dental treatments only. You must pay out-of-pocket for a qualifying treatment to be eligible. Qualifying services include, but are not limited to:

  • Routine cleanings and preventive care
  • Fillings, crowns, and restorative procedures
  • Teeth whitening and cosmetic treatments
  • Dental implants and oral surgery
  • Orthodontic treatments (braces, Invisalign, etc.)
  • Periodontal (gum) treatments
  • Root canals and endodontic procedures
  • Dentures and prosthetics
  • Extractions and emergency dental care

2.1 Services That Do NOT Qualify

The following services do not qualify for the Smile & Save Reward:

  • Dental examinations (new patient exams, comprehensive exams, periodic exams)
  • Consultations and treatment planning sessions
  • X-rays, imaging, or diagnostic services when not accompanied by a qualifying treatment
  • Any service paid for using dental insurance (private or government-funded)
  • Services provided at no cost or fully covered by insurance

Important: The service must be paid out-of-pocket by the patient. Insurance payments, including private dental insurance, do not qualify for this Program.

3. Eligibility Requirements

To be eligible for the Smile & Save Reward, Members must meet all of the following criteria:

3.1 Self-Pay Dental Treatment

The Member must complete and pay out-of-pocket for at least one qualifying dental treatment at a dental practice found through DentistsRanked.com. Payment must be made directly by the patient using personal funds, health savings account (HSA), or flexible spending account (FSA). Insurance payments do not qualify — the patient must pay the full cost of the service themselves.

3.2 New Patient Status

The Member must be a first-time patient at the dental practice where the service is rendered. Individuals who have previously received treatment at the same dental office, clinic, or any of its affiliated locations are not eligible, regardless of when the prior treatment occurred.

3.3 Online Booking Requirement

The appointment must be initiated through the DentistsRanked.com website or associated digital platforms. Appointments booked directly with the dental office via telephone, walk-in, or third-party scheduling platforms are not eligible for this Program.

3.4 Age Requirement

The Member must be at least 19 years of age on the date of the dental appointment. Proof of age may be requested during the verification process.

3.5 Payment Method Restrictions

This Program is for self-pay patients only. Appointments paid using any form of dental insurance are excluded from this Program. This includes, but is not limited to:

  • Private dental insurance plans (employer-sponsored or individual)
  • Dental discount plans or savings programs
  • Medicaid or state Medicaid programs
  • Medicare dental coverage
  • Children's Health Insurance Program (CHIP)
  • Veterans Affairs (VA) dental benefits
  • TRICARE dental programs
  • State or provincial health plans (including Canadian provincial health insurance)
  • Canada Dental Care Plan or similar government programs
  • Any other insurance or third-party payment arrangement

To qualify, you must pay the full cost of your dental treatment out-of-pocket. If any portion of your treatment is covered by insurance, you do not qualify for this Reward.

3.6 Claim Frequency Limitation

The Program is limited to one Reward per person every 18 months. Members who have previously claimed a Reward under this Program, or any predecessor or similar program operated by Dentists Ranked, within the preceding 18-month period are not eligible to claim an additional Reward.

3.7 Name Consistency

The name used to book the appointment through DentistsRanked.com must match the patient name on the dental receipt. If multiple appointments are booked under a single account for different individuals, only one person may claim a Reward per qualifying appointment.

3.8 Personal Treatment

The Member must have personally received the dental treatment. Booking an appointment for another person (such as a family member) does not qualify the account holder for a Reward unless they themselves received and paid for dental treatment.

4. Reward Claim Submission

4.1 Submission Deadline

Members must submit their claim within 90 days of the dental appointment date. Claims submitted after the 90-day period will not be honored, and the offer will be considered expired.

4.2 Required Documentation

To claim your Reward, you must submit a valid, itemized dental receipt through our online claim form. Acceptable documentation must include:

  • Date of the dental appointment
  • Name and address of the dental practice
  • Name of the patient (must match booking name)
  • Description of qualifying treatment rendered (not just an exam)
  • Total amount paid out-of-pocket by the patient
  • Proof that no insurance was used (receipt should show self-pay or cash payment)

Not Accepted: Treatment plans, cost estimates, appointment confirmations, scheduling emails, insurance Explanation of Benefits (EOB) statements, or receipts showing insurance payments are not accepted. Receipts for exams or consultations only do not qualify.

4.3 Verification Process

Dentists Ranked reserves the right to verify all claims. We may contact the dental practice to confirm the appointment occurred and that the service was rendered and paid for. Submitting fraudulent, altered, or falsified documentation will result in immediate disqualification and may result in a permanent ban from future participation.

5. Reward Fulfillment

5.1 Delivery Method

Upon successful verification of your claim, you will receive a $60 Amazon Gift Card code via email. The email will be sent to the address associated with your DentistsRanked.com booking. Please ensure your email address is accurate and check your spam/junk folder.

5.2 Delivery Timeline

Rewards are typically delivered within 5-7 business days of claim approval. During high-volume periods, delivery may take up to 14 business days. If you do not receive your Reward within 14 business days of claim submission, please contact us at hello@dentistsranked.com.

5.3 Gift Card Terms

The Amazon Gift Card is subject to Amazon's terms and conditions. The gift card code can be redeemed at Amazon.com (US) for eligible purchases. Dentists Ranked is not responsible for gift card expiration, loss, theft, or unauthorized use after delivery. Gift cards are non-refundable and cannot be exchanged for cash or other forms of compensation.

5.4 No Cash Alternative

The Reward cannot be redeemed for cash, check, direct deposit, or any payment method other than the Amazon Gift Card. The Reward value ($60) is fixed and cannot be adjusted based on the cost of dental services received.

6. Exclusions and Limitations

In addition to the eligibility requirements above, the following exclusions apply:

  • This Program cannot be combined with other promotional offers, discounts, or reward programs offered by Dentists Ranked or participating dental practices unless explicitly stated.
  • Dental services rendered outside the United States are not eligible.
  • Services provided by dental schools, teaching clinics, or community health centers operating under government funding may not qualify.
  • Dentists Ranked employees, contractors, and their immediate family members are not eligible to participate.
  • Claims submitted with incomplete, illegible, or unclear documentation may be denied.
  • Dentists Ranked reserves the right to deny claims that appear suspicious, fraudulent, or that otherwise do not comply with these Terms.

7. Program Modifications and Termination

Dentists Ranked reserves the right to:

  • Modify the Reward amount, eligibility requirements, or other Program terms at any time
  • Suspend or terminate the Program at any time without prior notice
  • Disqualify any participant who violates these Terms or engages in fraudulent activity
  • Limit participation in any geographic area or for any dental practice

If the Program is terminated, claims submitted prior to termination will be honored provided they meet all eligibility requirements.

8. Tax Responsibility

Recipients of the Reward are solely responsible for any federal, state, or local taxes that may apply to the receipt of the gift card. Dentists Ranked does not provide tax advice. We recommend consulting with a tax professional regarding any tax implications of receiving this Reward.

9. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, DENTISTS RANKED SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATING TO THIS PROGRAM, INCLUDING BUT NOT LIMITED TO:

  • Technical failures or errors in claim submission
  • Delays in Reward delivery
  • Lost, stolen, or unredeemed gift cards
  • Actions or omissions of participating dental practices
  • Quality or outcome of dental services received

This Program is a promotional offer and does not create any contractual obligation for Dentists Ranked to provide Rewards beyond the terms stated herein.

10. Dispute Resolution

Any disputes arising out of or relating to this Program shall be governed by the laws of the State of Delaware, United States, without regard to conflict of law principles. By participating in this Program, you agree to resolve any disputes through binding arbitration in accordance with the terms set forth in our Terms of Use.

11. Privacy

Information collected in connection with this Program will be handled in accordance with our Privacy Policy. By submitting a claim, you consent to the collection and processing of your personal information for the purpose of administering this Program.
