No. This promotion is for self-pay patients only. You must pay out-of-pocket for your dental service to qualify. Payments made through dental insurance, including private insurance plans, do not qualify for this reward.
After your appointment, simply take a photo or scan of your itemized receipt showing the date, dental office name, services rendered, amount paid, and patient name. Then submit it through our online claim form. We'll verify the details and send your gift card within 5-7 business days.
You must submit your receipt within 90 days of your dental appointment date. After 90 days, the offer expires and cannot be claimed.
Yes! You can claim a new reward every 18 months. Just book through DentistsRanked.com, visit a new dentist (one you haven't been to before), pay out-of-pocket, and submit your receipt.
